Abstract
Abstract : Recent published literature on error prevention emphasizes the use of technology to reduce the potential for medication errors. Computerized prescriber order entry, chemotherapy-specific software and computerized clinical decision support systems are a few of the technologies being advocated to promote patient safety. Although the current focus of error prevention is use of technology, simple and easily implemented strategies to reduce chemotherapy errors should not be overlooked. These strategies include eliminate the use of acronyms, standardize prescribing language, multidisciplinary monitoring of medication use and verification, provision of up-to-date information at the point of care as well as consistently use a reliable method to verify patient identity. Pharmacists should take the lead in implementing policies that will minimize the risk of future errors. Paragraph Headings: 1. Summary Tables: 1.
