Abstract
Objective: To analyse the clinical data of orthotraumatologic injury patients in road traffic accidents (RTA) admitted to Emergency Department of C.H.C.S.J of Macau, and interpret the injury mechanism and epidemiologic features. Methods: A retrospective clinical study. Totally 1802 participants were patients admitted to Emergency Department of C.H.C.S.J with RTA during October 1, 2003 to June 30, 2004. Results: (1) Sex ratio in 1802 RTA injuries: M 58.8%, F 41.2%. (2) Age distribution of RTA concentrated from 15 to 45 years old mainly, mean age is 30.5 y, a peak distribution was found in patients between 16-21 y. (3) Arrival time concentrated in busy traffic time, 08:00-09:00 and 13:0-15:00. (4) Fatality rate of RTA injury patients was only 0.44% (8/1802). (5) 13.0% (235/1802) were orthotraumatologic cases. (6) Total hospitalized orthotraumatologic cases wsa 31.5% (74/235), mean hospitalized duration was 10 days; Discharged cases with outpatient department follow-up was 42.2% (68/161). (7) Among total RTA and fracture 10.1% (182/1802), but among orthotraumatologic cases, fracture was 77.4%(182/235), joint dislocation 4.26% (10/235), and joint sprain 16.6% (39/235). Conclusion: (1) Fatality rate in RTA and fracture rate were low, reasons may be low driving speed due to narrow streets, crowded pedestrians and no highway in Macau, so less major RTA happened usually. (2) A peak age distribution found in patients between 16-21 years old, probably caused by young motorcycle drivers. (3) Arrival time indicated RTA happened time indirectly, which consists busy traffic time in Macau. (4) In orthotraumatologic cases, fracture was 77.4%, usually happened in some serious TA cases, considering caused by direct collision and squeeze. (5) In fracture & sprain cases, lower limb > upper limb, because of direct collision or squeeze caused by motorcycles and cars; In joint dislocation cases, upper limb was the main component, because upper limb joints with wide range of movement, usually caused by strong pull, push and twist.
