Infected femoral pseudoaneurysms in intravenous drug addicts

Abstract
Objective: Infected femoral false aneurysm is a serious vascular complication in intravenous self-injection of drug. The purpose of the this study is to determine the outcomes for intravenous self-injection of drug patients with infected false aneurysm. Methods: There were 7 drug addicts presenting with 9 infected femoral pseudoaneurysms were treated in the Department of Surgery, Hospital Central Conde de S. Januario in Macao from October 1997 to December 1998. There are 4 men and 3 women, with ages from 15 to 31 years (mean 33.6 years). Positive intraoperative tissue cultures were seen in total cases, with 3 being pure growth of staphylococcus and 6 mixing bacteria. 3 pseudoaneurysms involved the femoral artery and vein treated with ligation of external iliac artery, vein and superficial femoral artery, vein. 2 of 3 pseudoaneurysms was revascularization of artery and vein, through an extra-anatomical iliac-femoral bypass with ringed polytetrafluroethylene(PTFE). The other was only performed an extra-anatomical artery bypass with ringed PTFE, because of thrombosis of vein. 4 pseudoaneurysms involved femoral artery and were performed an extra-anatomical artery bypass. All the groin aneurysms were dealt with after bypass. All the groin aneurysms were dealt with after bypass. After drainage of the abscess and control bleeding, the aneurysm sac was debrided and profunda femoral artery was ligated. All the patients was ligated. All the patients were managed with perioperative and postoperative systemic antibiotics to control infections. Results: There are no hospital mortality, no amputation in all of patients. There are no graft infection. All insicions were healed in 2 weeks. The duration of follow-up range from 1 to 12 months. 4 grafts were occluded during the follow-up period. Those patients suffered from intermittent claudication. Conclusion: The patency of iliac-femoral artery extra-anatomical bypass grafts in drug addict patients are lower than the patency of grafts in others disease.
