- Abstract
- Abstract : Objective: Hepatic hydatidosis often is complicated by biliary involvement. The aim of this study was to verify the value of ERCP. and ES. in preoperative detection and management of these complications. Methods: 47 patients affected by hepatic hydatidosis and with no radiological and biohumoral signs of biliary involvement, were selected and randomly divided into 2 groups. In group A patients no preoperative ERCP was performed; group B patients were diveded into 2 subgroups, B1 and B2, according to the presence or the absence of findings positive for papillitis or biliary tree hypertension. Group B1 a was divided into 2subgroups: in group B1b underwent surgical treatment without a previous ESA. Results: In group A 6 patients had an external biliary fistula in postoperative period, in 4 of them fistula was low-flow and closed up spontaneously after 7-23 days; in one patient bile leak was high-flow and resolved only after ES. Conclusions: The use of ERCP can be useful in hydatid liver disease even when it is asymptomatic for biliary signs and it may be followed by ES when biliary involvement is present. The subsequent surgical treatment will have a reduced postoperative morbidity, a shorter hospitalization period and lower costs. Paragraph Headings: 1. Objective 2. Materials and method 3. Results 4.
