Abstract
Objective: To study the significance of applying atropine test to the diagnosis of the process from deep coma to brain death and come up with the accurate dosage of atropine in the diagnosis of brain death. Methods: We made a clinical research on 36 cardiopulmonary arrest patients who resumed their heart beat and kept mechanical ventilation after CPR. In these patients, the GCS was in the range of 3-5. We made atropine test every 4 hours. If two successive atropine tests were negative after the intravenous injections of atropine 1 mg, the following two tests would use 2mg and 5mg atropine respectively. Meanwhile, we evaluated their GCS change and their brain stem reflections. The electrocardiogram and blood pressure were continuously monitored after implementation of the atropine test. Results: We found that: comparison of atropine test on the heart rate changes between brain death group and deep coma group showed significant difference. The increase of heart rates in brain death group after the atropine test was not affected by the difference in dosage. Conclusion: These findings suggested that the atropine test culd be used as one of the index to make a differential diagnosis of deep coma and brain death, and the repeated atropine test in the dose of 1 mg at 4-hours interval could be regarded as one of the criteria in the diagnosis of brain death if it resulted negative.
