- Abstract : Choledochal cysts are uncommon anomalies of the biliary system and are probably congenital in origin. They are manifested by cistic dilatation of the extra or intrahepatic biliary tree or both. Todani's classification divides choledochal cysts in five main types. Common symptoms and signs include right hypocondrial or epigastric pain, nausea, vomiting, a history of obstructive jaundice or pancreatitis. Malignancy is the most dangerous complication. ERCP or cholangiography are the most reliable diagnostic techniques in the investigation of patients suspected having choledochal cyst, because they give an accurate demonstration of both the biliary and pancreatic ducts. The authors present a rare case of choledochal cyst in a 22 year old man who bad a 10 year history of intermittent epigastric pain. Altough the use of various techniques like endoscopy, CT scan, Cholangio-CT scan, Cholangio-CT and ERCP, diagnosis was established very hardly. Only during the operation a diagnosis was made. The gold standard in the treatment of choledochal cyst is complete excision because malignant change has not been observed after its total or subtotal excision. Paragraph Headings: 1. Introduction 2.
