Abstract
Objective: In several case reports, distension and irrigation of the uterine cavity during fluid hysteroscopy was suspected to cause tumor cell dissemination into the abdominal cavity in patients with endometrial carcinoma. It is the aim of this study to compare the incidence of positive peritoneal cytology in patients who underwent dilatation and curettage (D & C) with or without previous hysteroscopy. Method: A retrospective study was conducted on eighteen patients with endometrial carcinoma who were recruited from 2002 and 2006. Endometrial carcinoma was confined to uterus without macroscopic extrauterine disease. Fluid hysteroscopy was performed by using 5% glucose solution irrigated at a final flow of 150 ml/min with a intrauterine pressure 120 mmHg. Positive peritoneal cytology was considered the primary end-point of this analysis. Results: 5 patients (27.8%) had diagnosis of endometrial cancer made only by D&C, 13 (72.2%) underwent hysteroscopy and then D&C. Peritoneal cytology was all negative in two groups. One case died from peritoneal recurrence 4 years after operation. Conclusion: The comparison of cytological findings from the peritoneal lavages after hysteroscopy and D&C did not show a statistically significant difference. We assume that hysteroscopy does not increase the risk of dissemination of tumor cells into the peritoneal cavity. Paragraph Headings: 1. Introduction 2. Method 3. Results 4. Discussion 5.
