Abstract: Abstract : Primary tracheal tumors are rare and the presenting symptoms are common and nonspecific, which often overlooked until they reach to an advanced stage. Unfortunately, the chest radiograph is often normal. It has been common to see patients with indolent tracheal neoplasms even dyspnea presents for months or even years without suitable investigation or therapy until hemoptysis leads to a diagnostic bronchoscopy. Here, a case with tracheal poorly differentiated adenocarcinoma, presented with dsypnea half year but it has not lead to clinical attention. The patient was diagnostic as tracheal neoplasm until hemoptysis and severe dyspnea presents. When the tracheal obstruction of neoplasm is about 90% and dyspnea progressively worsen in short time. The urgent surgical management of tracheal neoplasm to resolve asphyxia has done local invasion of the neoplasm has occurred. Paragraph Headings: 1. Case Presentation 2. Disucssion 3.
