Abstract
Objective: Anastomotic dehiscence is an important cause of morbidity and mortality after anterior resection for rectal cancer. Regarding this problem, the use of a protective defumctioning stoma still remains a point of debate. Methods: Fifty-eight patients with rectal cancer between 3.5 to 15 cm from the anal verge underwent anterior resection and total mesorectal excision. A protective colostomy was performed in forty-four patients according to the geight and the type of anastomosis. Results: In this study of fifty-eight patients, two had clinically evident dehiscences and two radiological ones with an overall incidence of 7%. Conclusions: It is not necessary to construct a protective colostomy for all anterior resections but it can be beneficial in selective cases. The parameters useful in deciding when to apply a protective colostomy are those that most influence the incidence of anastomotic dehiscence: height of the anastomosis from the anal verge, type of anastomosis, preoperative radiotherapy and local risk factors.
