Abstract
Objective: To review the suspicious symptoms and signs of appendicitis in surgical dept to interpret the value of using ultrasound as an adjuvant diagnostic method. Methods: The clinical data of acute appendicitis patients who reveived appendectomy from August, 2002 to 2004 were analyzed respectively. We reviewed retrospectively the clinical records and imaging findings of 217 patients with clincal suspicion of acute appendicitis. 145 patients were operated upon clinical evidence alone (group I), while 72 were also referred for US subsequently (group II). Sensitivity, specificity, accuracy rate in group I, negative appendectomy rate and positive predictive rate between two groups are also been noted. Furthermore, we also analysed the sensitivity of clinical findings in clinical suspected appendicitis. Result: Clinical evidence that alone clinical diagnosis of appendicitis (group I) occupied 66.8% (145/217), combined with intervention of echo (group II) was 33.1% (72/217). According to the diagnosis of post-operation of pathological results, negative appendectomy in group I was 14.5% and 1.5% in group II, positive predictive rate were 85.5% and 98.1%, respectively. Total 72 patients had preoperative US, the sensitivity was 80%, specificity 83%, accuracy rate 79.2%. Conclusion: Intervention of US in diagnosis of acute appendicitis had the benefit to diagnosis of appendictitis, particularly in clinical case. Paragraph Headings: 1. Introduction 2.
